Consent and Opt-In: By providing your phone number and opting in to receive text messages, you consent to collecting and using your personal information as described in this policy. Privacy: City Hydration will maintain the confidentiality of the patient's personal health information following applicable laws and regulations. No mobile information will be shared with third parties/affiliates for marketing/promotional purposes. The above categories exclude text messaging originator opt-in data and consent; this information will not be shared with third parties.
Contact Information: Patients agree to provide City Hydration with their current mobile phone numbers and must be notified of any changes to their contact information.
Termination: Patients may terminate this consent at any time and for any reason, including but not limited to changes in the law or changes in services provided by City Hydration.
